The forms below should be used for CSA and other university reporting obligations
Campus Security Authority Reporting
Submit a report of a Clery Act crime as soon as possible and within 24 hours of receiving the information.
Child Abuse and Neglect Reporting
Report suspected child abuse or neglect on university owned or controlled property or during university programs or activities.
Clery Act Travel Reporting
Report university-sponsored student travel involving overnight stays and university-controlled lodging or academic spaces.
Dean of Students Office Reporting
Report a violation of the student code of conduct.
Discrimination Reporting
Submit a report for acts of discrimination and/or harassment in potential violation of the university’s Nondiscrimination and Anti-harassment Policy.
Fire Reporting
Report to Risk Management by email that you have received information about a fire that occurred within the university's Clery geography.
Sexual Misconduct Reporting
Submit a report involving sex-based discriminatory conduct, including sexual assault, dating violence, domestic violence, and stalking.
UAPD Online Crime Reporting
Certain crimes can be reported online including annoying/harassing communications, graffiti/vandalism, and property-related crimes. For emergencies, call 9-1-1.
